Please complete all applicable forms prior to your appointment. Forms must be received before your session can begin, and completing them in advance allows us to protect the full length of the time you've reserved for your care.
Taking a few moments to share this information beforehand means that when you arrive, there is nothing left to manage.
You can simply settle in, exhale, and receive.
